Steps to reproduce

I have tested this issue on four different machines with slightly different configurations and they have returned slightly different results but all indicative of PDF printing issues.

When opening if-request-form-20150902-2.pdf in Microsoft Edge on Windows 10, while the document looks correct in the browser, two different issues have been discovered when subsequently printing the PDF.

“IFT Blank Second Page.pdf” is a scan of what was printed on three separate machines - note that none of the text on the second page was printed. This occurred whether or not Adobe Acrobat was installed, and even occurred after opening the PDF in Edge then saving locally and opening that file in Adobe Acrobat - indicating that the file was saved by Edge after some kind of internal modification, rather than just saving the document as retrieved from the webserver.

“IFT Gobbledygook.pdf” is a scan of what was printed on a single machine that did not have Adobe Acrobat installed.
